We are looking for a compassionate and organized Nurse Navigator to join our healthcare team. The Nurse Navigator will serve as a patient advocate and liaison, helping individuals and their families navigate the complexities of the healthcare system. Key duties include assessing patient needs, developing individualized care plans, facilitating communication between patients and providers, and tracking patient progress. You will also play a critical role in educating patients about their diagnoses, treatment options, and follow-up care, empowering them to make informed decisions about their health.
This position requires a registered nursing license and experience in case management, oncology, cardiology, or another relevant specialty. Certification as a Nurse Navigator or in a related field is preferred but not required.
